Fall Prevention

We perform a Home Safety Evaluation to identify risk factors in the home and implement safety measures for a safe living environment. Our therapists develop home exercise programs to increase balance and strength to prevent falls or injury.

Medication Management

Our nurses work closely with patients and physicians to achieve desired medication effects and prevent interactions. We provide education and develop medication schedules and weekly planners to increase adherence.

Wound & Post-Operative Care

We provide quality wound care with the most innovative supplies in the industry. Our nurses receive extensive training in Negative-Pressure Wound Therapy, and our program has been successful in healing chronic wounds.

Preventing Hospitalization

Our clinicians make every effort to prevent re-hospitalization through education, medication reconciliation and in-home procedures, maintaining lower re-hospitalization rates than other home health agencies nationwide.

Chronic Care

We specialize in individualized care plans to manage new or chronic illnesses such as Alzheimer's, arthritis, congestive heart failure, COPD, diabetes and dementia, using a multidisciplinary and holistic approach.

Do I qualify for services?

Eligibility

  • You must be under the care of a physician who orders home health.
  • You must be in need of skilled care.
  • You must be home bound. This is met if you leave home infrequently, with difficulty and taxing effort. It does not mean you can never leave your home.
