Do I qualify for services?

  • You must be under the care of a physician who orders for home health.

  • You must be in need of skilled care.

  • You must be home bound (This requirement is met if the patient leaves home infrequently with difficulty and taxing effort. It does not mean you cannot ever leave your home.).
