The Agency’s mission is to provide professional and paraprofessional services to clients in their homes assisting them to achieve the highest level or potential in their day-to-day self-care activities. We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional’s point of view.

Philosophy.

BIO HOME HEALTH SERVICES, INC. goals and services are based on two fundamental philosophical principles: the belief in the innate worth of the aged and disabled individual and the belief that each individual, regardless or age, race, color, creed, sex, national origin or handicap(s) is entitled to maximize his potential as a human being and as a member of society.

It is the contention of BIO HOME HEALTH SERVICES, INC that the aging process is a normal state in the development of any individual and that chronic disease and disability are, to some degree, a part or that process. This Agency is dedicated to rehabilitating aged and disabled individuals within the confines of their residence, in order that they may maximize their contributions and fulfill their goals as a family member and member of society with a minimum of conflict. In accomplishing this end, it is felt that the self-respect of the individual can and will be enhanced.

Services Are Performed by Our Dedicated Team of Health Care Professionals

Skilled Nursing

Registered Nurses and License Vocational Nurses are available to assist with all nursing needs, including skilled nursing teaching, observation, assessment and procedures.

Physical Therapy

Licensed Physical Therapists are available to establish a program of therapeutic exercises and to educate in the proper use of medical equipment and assistive devices.

Speech Therapy

Speech Therapists are available to assist with a program of speech and language instructions to improve and restore communicative skills which may have been lost due to decease or accident.

Occupational Therapy

Registered Occupational Therapists are available to assist our patients in the restoration of functional abilities such as feeding, grooming, dressing and home making as well as recreation and vocational skills.

Medical Social Services

Medical Social Workers are available to assist patient and family with problems related to care at home and to coordinate essential community services, help establish financial assistance and other services needed on the home.

Home Health Aide

Certified Nurse Aides are available to assist with personal care, simple treatments, exercises and personal hygiene.

Our highly trained clinicians specialize in

Fall Prevention

Our medical professionals will perform a Home Safety Evaluation to identify risk factors in the home.  Changes in health conditions, advancing age and certain medications can increase the risk of falls.  Safety measures will be implemented to achieve a safe living environment.  Our therapist will also develop a home exercise program to increase balance and overall strength to prevent falls or injury for those in need.

Medication Management

Our nurses work closely with patients and physicians to achieve desired medication effects and to prevent medication interactions.  Education will be provided on therapeutic effects, adverse effects and common side effects of all medications.  Testing for therapeutic levels can also be performed as requested by physicians.  Our nurses will develop medication schedules or weekly planners to increase adherence to medication regimens.

Wound Care / Post-Operative Care

Our nurses provide quality wound care with the most innovative supplies in the industry.  Our nurses received extensive training on Negative-Pressure Wound Therapy and wound care techniques throughout the year.  Our excellent Wound Care Program has been successful in healing of chronic wounds.

Preventing Hospitalization

Our clinicians will make every effort possible to prevent rehospitalization.  Education, medication reconciliation and certain procedures provided in the home have proven to be effective in preventing rehospitalizations.  We have maintained lower rehospitalization rates among our patients compared to other home health’s in the nation as supported by Kinnser RiskPoint.

Alzheimer’s / Arthritis / Congestive Heart Failure / COPD / Diabetes / Dementia

Our dedicated clinicians specialize in individualized care plans to manage new or chronic illnesses.  We provide thorough and innovative information on illness management.  Optimal independence will be achieved with the participation of patients and caregivers.  Our desire is for patients to achieve the highest quality of life possible using a multidisciplinary and holistic approach.

Am I Qualified?

Qualification

  • Must be under the care of a physician who orders for home health
  • Must be in need of skilled care
  • Must be home bound

Home bound: This requirement is met if the patient leaves home infrequently with difficulty and taxing effort to do so. (doesn’t mean you cannot ever leave your home)
